To remove a Data Form from Settings, delete all Field ID rows, then click the Delete Button. 

NOTES: 

• A Data Form must be unlocked on the list view in order to delete a field.
• This will remove the Data Form from each contact. 
• Custom and Saved Reports will still pull information from deleted Data Form Field IDs. 
• To restore a Data Form, have your System Administrator create a ticket on this website.
