Discharge client

When you are ready to discharge a client there are a couple of items that you need to ensure the client has first. This includes a date of admission and an active program. To discharge a client you will need to follow the steps below in the order they are listed.

Episode tab

First, you will need to discharge the client from their current program/level of care. You can do that by following the instructions below:

  1. Navigate to the discharging client's chart.
  2. Navigate to the "Episode" tab.
  3. Select the client's current program (level of care).
  4. Enter the "End Date" and, if desired, the "End Time."
  5. Select a "Discharge Reason." 
     Note:  The discharge reasons are customizable to fit your organizations needs.
  6. Select "Save Program Data."

Now that you have discharged the client from their current program (level of care), you will discharge and inactivate the client. You can find the instructions for that below:

  1. Navigate to the "Edit Client" screen.
  2. Enter the date of discharge in the "End Date" field. This will generate a length of stay in the "Length/Stay" field.
  3. Remove the "Current Active" tag and add select the "Past Discharged" tag.
  4. Select the "Inactive" checkbox located at the bottom left corner.
  5. Select reason for inactivation. To add additional options to this drop-down, send a ticket to team@bestnotes.com with those reasons.
  6. Select "Save."
  7. You will notice at the top of the client chart it says "INACTIVE" with the reason in red text. This is a discharged chart.

Med/Clinical

Now that you have successfully discharged the client, when you open the "Discharge Summary" the end dates and length of stay will pull into that discharge summary. See the instructions below for more details.

  1. Navigate to the "Med/Clinical."
  2. Navigate to the appropriate "Discharge Summary."
  3. Fill out any missing information in the discharge summary, and review the document.
  4. Once you have completed reviewing and filling out the "Discharge Summary" add any necessary signers and select "Publish Snapshot."

Signatures

You will then need to ensure that the appropriate signatures are collected on the document. Once that is completed, you will have successfully discharged your client!
