Limited customization

On the first page of the common application within the "Client Information" section, the First Name, Last Name, and Date of Birth are all required fields (see Figure 1-1).

You can choose to either show "Height" and "Weight", or you can hide these questions (see Figure 1-2). These two questions are considered a single unit when deciding to show or hide them.

You can choose to show all of the following fields, or to hide them all: "Eyes", "Hair", "Shoes", "Waist", and "Shirt" (see Figure 1-3). These 5 questions are also considered a single unit when deciding to show or hide them.

Within the "Educational Information" section, you can choose to hide two questions (see Figure 1-4). These will be shown and hidden together, and cannot be hidden independent from each other.

Within the "Educational Information section", two more questions can be hidden, but you can choose one or both of these questions to hide (see Figure 1-5).

Within the "Psychological Continued" section, you can choose to hide one question (see Figure 1-6).

Other than the items discussed here, the common application cannot be modified. If you or another company chooses to hide these questions, upon transferring the application, these questions will not have a response displayed. Also, any modifications you make are on a company basis, and cannot be customized per user.
