Once a survey is submitted, OutcomeTools scores it and shows the full results in a locked-down view of the original survey, with an alert banner at the top if the submission triggered one.
How to open a completed survey for review
- Open the client and go to the Surveys tab.
- Switch to Completed Surveys.
- Click the survey name in the Survey column to open the review modal. (Clicking elsewhere on the row opens the instance details, where you can adjust delivery and other settings if needed.)
You can also open a review from the account-level Surveys tab — anywhere a completed survey name is listed as a link.
What the survey review shows
- Score bar (top) — the overall score for the instrument and, if the instrument has subscales, a cell for each subscale with its own score. Scores are shown as numeric values.
- Item responses — every question and the respondent's answer, rendered in the original survey layout but locked (no editing).
- Summary / description (if the instrument defines one) — shown in the header area of the survey content.
- Alert banner (top of the modal, if applicable) — if this submission triggered an alert, a banner appears at the top of the review showing the matched band label and severity.
How score bands work
Bands are the interpretation ranges defined by the instrument (e.g. PHQ-9 total 0–4 = Minimal, 5–9 = Mild, …). OutcomeTools evaluates them automatically during scoring: a band that is configured to raise an alert produces the alert banner at the top of the review, with the band's label (such as Moderate) and severity. The score bar itself displays the numeric scores; an instrument that defines no bands simply has none to evaluate.
How to see a client's score trend over time
The review modal shows a single submission. To see this client's scores for an instrument across time, open the client's Summary tab, where each instrument has its own trend chart.
