The Library shows every instrument your account is subscribed to. Admins can add instruments from the available catalog and customize each one so the title, description, and alert behavior fit how your team uses it — without touching the underlying instrument.
Where do I customize a survey?
Open Library from the navigation. Each row has an edit button on the left — click it to open the Customize modal for that instrument (titled Customize: PHQ-9, using the instrument's short title). Clinicians (Level 1+) see the same Library list and can preview an instrument by clicking the row, but they don't see the edit button and can't open this modal.
Rows with active customizations are flagged with a small C chip next to the title; archived instruments show an archived chip.
What can I customize on an instrument?
Customizations are scoped to your account only. The shipped instrument is unchanged — your edits live in your account's library and override the default at runtime.
- Active status — whether the instrument appears in the assign-survey picker.
- Title — the long, human-readable name shown to clients and clinicians.
- Description — the longer copy that appears with the instrument.
- Built-in alerts — disable any alert the instrument ships with.
- Custom alerts — add your own alerts on top of (or in place of) the shipped ones.
The short title (e.g. PHQ-9, shown in the ID column) is the instrument's shared identifier and is not customizable.
How do I add a survey to my Library?
Click the + button in the Library toolbar to open Add Survey to Library. Search the available catalog by name, filter by category, Preview an instrument, and click Add to add it to your account's Library — added surveys show an Added chip. Which instruments appear in the catalog is governed by survey access: surveys published openly plus any granted to your account. If an instrument you need isn't listed, contact OutcomeTools support to have it granted.
How do I activate or archive an instrument?
The footer of the Customize modal has an Active switch. Set it to Active to make the instrument available for assignment, or Inactive to archive it. Archived instruments still show up in the Library when Show Archived is on and their historical data is preserved, but they can't be assigned to new clients.
How do I override an instrument's title and description?
Each field shows the shipped default above the input. Type to override; press Reset to default to clear your override and fall back to the original. Leaving the input blank also means "use the default."
What you set here is what clients and clinicians see — on the survey page, in the Library, and in reports.
How do I disable a built-in alert?
Alerts shipped with the instrument are listed read-only with a checkbox. Uncheck one to disable it for this account — submissions that would have triggered it simply don't.
You can't change a built-in alert's thresholds, message, or category. To tighten or relax one, disable the built-in and add a custom alert that targets the same field or score with the band you want.
How do I add a custom alert?
Click the + next to Custom Alerts to add one. Each custom alert fires when a response (or a score) falls into a band you define. In the editor you choose:
- Scope — Field response or Score.
- Target — a specific field, the overall score, or a subscale.
- Category and Message — what shows up on the alert in the client's record. The Insert button next to the message drops in placeholders like
{{trigger_label}}and{{trigger_severity}}that get filled in from the matched band at submission time. - Trigger bands — one or more value ranges, each with a label and a severity (0–10).
Custom alerts behave exactly like built-ins: they create rows in Alerts, route to notification recipients, and queue integration events.
Saving customizations
The Save button stays disabled until you change something. Saving commits all sections at once — status, title, description, disabled built-ins, and custom alerts. Closing with unsaved changes prompts before discarding.
